What to check before you choose
- Scope: every place with ePHI
- The analysis must cover all ePHI the organization creates, receives, maintains or transmits — systems, devices, vendors and locations (164.308(a)(1)(ii)(A)).
- Output you can act on
- Documented threats, likelihood, impact and risk level, and a plan for the measures you choose (164.308(a)(1)(ii)(B)).
- Keeping it current
- The rule sets no fixed interval; review it periodically and after changes. Check that the tool makes an update easy rather than starting over.
- Free tool or paid guidance
- The HHS SRA Tool is free. Paid tools add guidance, remediation tracking and expert help.
- No PHI needed
- A risk analysis describes systems and risks, not patients. Do not enter patient information into the tool.
- MIPS is a separate question
- Medicare MIPS asks clinicians to attest to a security risk analysis; the HIPAA duty applies whether or not you report MIPS (MIPS security risk assessment).
Common mistakes
- Answering a questionnaire without first listing systems, devices and vendors.
- Finishing the analysis and never acting on the risks it found.
- Not updating it after a new system, a move or a breach.
